Thank you for your interest in MHHNP. If you would like to become a network member, please follow these simple steps:
- View the criteria to join Memorial Hermann Health Network Providers.
- If you feel you meet these criteria please download and complete the Pre-Application form.

Our staff will review your Pre-Application form, and after verifying that you meet the criteria for membership, will mail you an application packet to begin the credentialing process.
Once you receive the application packet, please download the Texas Common Credentialing Application , complete the application and return it with the additional documents requested in the packet.

Upon approval by the MHHNP Credentialing Board you will be sent a welcome packet notifying you of your acceptance and giving you access to over 20 different health plan contracts.
